Medical Ethics Question

You are a doctor, and you are in charge of deciding who gets a liver transplant. You have two patients in need of a liver. 1, a 65 year old man in excellent health, or 2, a 22 year old woman with Hepatitis B and a history of IV drug use.
Who do you give the liver to?
(I’ve heard that this is a common medical school interview question.)
Edit: To clarify, the man needs a liver, but is in otherwise excellent condition.

22 year old with hepatitis since hepatitis affects the liver. A 65 year old in excellent health wouldn’t need a liver.

Is there a right or wrong answer? I'm giving it to the healthy 65yo because IV drugs are trashy and the 22yo will end up OD'ing on fentanyl anyway. Not the med school answer but I'm not a doctor.

Well the 65 year old isn’t in excellent health if he needs a liver transplant.
Does the 22yo still use drugs or alcohol? If so, she is not getting the liver.

When dialysis was relatively new in the 60s, and there were just a few machines, Seattle had a "God Committee” of regular citizens who assessed applicants and decided the few who would get the life-saving treatment.

But in a vacuum, you need more info on the drug use. Is he clean? If so how long? Liver transplants are pretty common with chronic Hep B so that isn't always disqualifying, it depends on the severity of the disease.
Second I would want to know why the liver failed in the man. They don't really just quit working in an otherwise healthy individual.
As stated the chances of the 22yo being disqualified are much higher than the 65yo.

Sounds like one of those questions where the necessary info to make a "correct" decision isn't given, and the panel just wants you to pick one and defend it when challenged.
