It was either butt sex or shooting cocaine up her anus.

It’s the shame or stigma of the type of cancer. Not cancer in general. It’s a good thing for her to discuss as people need awareness. As a family member of multiple types of cancer any discussion for awareness is good.

Coming up in the south, many people called making a spectacle of yourself "showing your arse". I believe that figure of speech is somehow appropriate here.

Used in a sentence:
"Stop showing your arse, Marcia. Nobody cared what kind of cancer you had until you brought it up."

I have no clue about anal cancer and the stigma associated with it but I can say that I feel uncomfortable discussing details about my rectal cancer even with my doctors. I do what I need to but it is not comfortable. I think it is a hold over from being raised that talking about problems with that part of the anatomy is not proper. I have always been very private about my toilet visits and to have to discuss in detail about pain, blood, mucus, stool consistency, color, and shape is not something that most folks are at ease with.
I can assure you that my rectal cancer did not come from anal sex mainly because I have never had anything shoved up my arse, before this diagnosis, and I think that the attitude of assuming that anal or rectal cancer is caused by sex is one of the reasons people do not want to say they have anal or rectal cancer.
just my 2 cents
