Sounds like he drowned and then was eaten. Jeremy Wade got into a swimming pool full of piranha and they showed no interest. He also swam with them in the wild and same result. But put some dead meat in the water and it’s a frenzy.

Considering his friends all swam to safety without incident, it's safe to say he probably drowned then ended up eaten by piranhas.

How does a farm lake get piranhas?
Also he would have died from drowning not a piranha attack. Piranhas don’t attack and kill live humans contrary to popular belief. One may bite you if food is scarce but typically they only eat on corpses on the river/lake bottom
