The virus attaches to the cell like pictured in the OP, then another spike comes out the middle and injects its DNA.

That DNA hijacks the cell and turns the cell into a virus-making factory reproducing thousands inside until the walls of the cell burst, thus scattering them out to find other similar cells.

Btw, viruses are specific to certain cells. It's like a lock & key type action, only able to attach to & hijack certain cells (say, the herpes virus family to gentitals/facial skin cells etc but can't hijack say, heart muscle cells)
