Hillary Clinton. Was lazy in starting her 1st campaign then lost all momentum to Obama before she got started.
Then took a job as Secretary of State and royally screwed herself because she couldn't take the job seriously. Even still she was smoking Trump who couldn't stop talking about his "Mexican judge" but decided to take the month of August off. Trump recovered in that time and the rest was history.

Even after losing to Obama she could have taken an 8 year vacation instead of pretending to be SoS and cruised to the presidency.

Runner up in recent times: Yevgeny Prigozhin

Built a private army and drove straight to Moscow. Could have easily taken it but made a deal and stopped. Died in a plane "accident" months later to the surprise of NO ONE.

I am surprised by the lack of mention of the American Retreat from Afghanistan three years ago. On an individual fafo achievement level the photos of the guys falling off the giant plane at 1000 feet is memorable. On a group fafo basis the killing of 13 young American troops left outside the security zone was awful military leadership. Abandoning 8 billion dollars of military equipment with no plan to disable/render useless was a military leadership fafo. Abandoning many thousands of local personnel that supported the us is the ultimate fafo
