Personally, I was adjacent to a situation where there was a death in a commercial accident. The decedent was a 65ish year old man. His adult children had already lived through the untimely death of their mother in 1995...when she was murdered by Tim McVeigh in OKC.

Not saying its impossible, but these restraints are installed in B&M coasters around the world with zero failures over decades.
Two completely independent hydraulic systems would have need to failed on the exact same ride.
Unfortunately in previous investigations generally the failure in other similar incidents generally occurs because the operators allowed them to ride even though the rider did not fit and the seat was not put into a locked position.
Obviously the investigation needs to play out, but this would affect coasters all over the world if the redundant systems failed.
Sort of like an airplane crash a lot of bad has to go wrong for something to happen

Pretty sure the last Rollercoaster death was a big fat black kid who couldnt fit into the shoulder harness properly. Pretty sure that park was shutdown.

I agree. Lookig at the pics, the lap bar was in the up position. Highly unlikely it just broke. Millions of riders using this system and never a failure. Possible but unlikely.
I am going with a series of things went wrong at the same time.
