First, let the clutch out, idle forward, pray the car in front moves again before I have to clutch in and brake to avoid a collision.

This. And depending....sometimes you can just start in second gear. You don't necessarily have to be in coast to do it. It depends on the slope and your abilities.

My way on way home from work in the hills of Tulsa was to pull over to a Happy Hour buffet and have a beer and food till the traffic thinned. Sure beat stopping/starting, rolling, clutching.

Rookie...You hold the cigarette in the same hand as the pop tart. That leaves your cigarette/pop tart wrist free to control the steering wheel while your right hand works the stick in between coffee breaks....jeez you guys need driving lessons!

Not too many, especially if you don't want the absolute base model. They're out there though. It's even harder if you need something bigger than a car though. Subaru offers the forester in multiple tiers. Mt isn't fully a second class citizen there. Almost all their vehicles come with a mt option.
