I do think that spouses should have a desire to smell good, be clean/hygienic, be healthy, be fit, be attractive to their SO's. And they should take care of themselves for their own sake, but also keep themselves fresh, clean, and healthy for their SO
I wouldnt want my wife having to do things to me if I were all fat, smelly, dirty and sloppy....thats degrading to her

Most us will hopefully get old at some point and your looks will absolutely fade. Worry about the things that you can control like your weight and hygiene

Yes,spouses have a duty to one another to remain as fit, healthy, and attractive as the inevitable encroachments of time allow. If one fulfills that duty and the other neglects it, there will be sexual dissatisfaction and likely infidelity. If both neglect it, there will be weekly trips to Walmart.
