Depends on state. In some states we have direct access. Usually much easier for your insurance if you just have an “eval and treat” prescription from your Dr.

Pretty much this. Such a therapist will actually attempt to increase motion in what they assess to be a restricted area of your spine not simply get a “pop” that will make you feel good for an hour and have you come back weekly for life. Many chiros will adjust people that actually have too much mobility. “Adjustments “are actually counterproductive.

There are ambulance chasing chiropractors. I dont know any ambulance chasing PTs. However, I go to both for "sports" injury type things (tendon/muscle imbalance issues). And honestly they do a lot of the same stuff. Chiro is more massage and "adjusting" with exercises added in. PT is more exercise with massage and adjustment added in.

There is a place for both us IMO. People swear by both so I think it is very personal preference dependent.

An outpatient clinic affiliated with a local hospital has a chiropractor on staff. I don't know how common this is but they make a point of advertising it.

A good chiropractor will incorporate physical therapy and massasge into their service, massaging muscles prior to the adjustment and providing a program of stretching exercises for after.

Mine did. My doctor wrote a prescription to the PT that outlined what the diagnosis and treatment goal was and how many visits he wanted the PT to give me before I returned back to the doctor for evaluation of progress and continuation or modification of the therapy plan, if needed. They very much operated as a treatment team to get me back up and running in the most effective way they could.
ETA: That was several years ago, though, and as others have pointed out, the laws in LA may have changed.
