Recently, my wife was ordering at the drive through of Jack N the Box. My 8 year old son was in the car with her. They asked my wife if she wanted toppings on the chocolate shake (for my son) and she replied 'Yes M'am'.
As she's pulling to the window, my son said 'Mommy, you called that man 'M'am'. As my wife was handing her card to pay at the window, she apologized for saying 'M'am', got a kind of awkward look, and then a few seconds later realized that it was actually a woman (i.e. wasn't immediately obvious upon appearance either).
