Well, on a serious note - your pal needs to find out if something happened - was he molested or raped? Is the son having identity issues - is there something he doesn’t like about himself? Does he need attention? Was he rejected by a girl or made fun of sexually?

If your friend never saw it coming, he has either had his head in the sand or the son is mentally ill. If he lived his entire life as a masculine male up to 25, he might be undergoing the initial “break” of schizophrenia or bipolar disorder. I have a kid with gender issues and it was not a surprise. My kid has been trying to reject their gender since they could speak. ETA just being real.

His son is 25. There's not much he can do. He does need to understand the chances of him burying his son due to suicide will increase exponentially if the son gets a sex change.

He needs to urge his son to seek therapy, thinking you are born in the wrong body is a learned behavior.
He should also make it clear to his son that he won't be paying for this insanity and that he will still always be considered his son (who mutilated his genitals), not his "daughter".
