My best and worst occurred at the same show. Tool Lateralus tour in '03(?) in Milwaukee. Tool's set was the best I have ever seen. Two hours of awesome music and absolutely stunning visuals.
Tomahawk opened for them. I had high hopes for them because I love Mike Patton and Faith No More, but that performance was absolutely terrible. Shitty sound mix, way offkey, whole band must have been hammered or something. Not even halfway through the crowd starts chanting "frick TOMAHAWK! frick TOMAHAWK!" And Patton really meekly squeezed out something like "So you wanna frick Tomahawk?" I don't think they finished their setlist before they were booed offstage.
Never had a shitty experience outside of just bland or bad performances. Guess I'm lucky.

Patty Smythe opened for Rod Stewart in the Superdome. I went with the wife because I wanted to see Patty Smythe.

Hmm... performance wise it would either be when I saw Everlast at Woodstock '99 or Counting Crows at Rock Fest '97 at Texas Motor Speedway.
Everlast was just horrible. His voice was shot to shite and you couldn't understand anything he was singing half the time. I've seen many an act that sounds very different from their studio albums but he was by far the biggest downgrade.
As for Woodstock itself? The price gouging sucked, the lack of functioning port-o-pots sucked, the lack of promised free water sucked, the lack of shade on the all concrete former Air Force base sucked, the money machines running out day 1 sucked, aggro bros going around and ripping women's tops off to grab their tits (like holy shite they gave no fricks) and starting fights w/people who didn't want to get forced into mosh pits sucked, and the police coming in and and beating the shite out of anyone that got near them causing the fires to get way out of control sucked. Other than that it really was a fun time.
Counting Crows was more of an issue w/them playing every single song slow as balls. They may as well have been putting on a set at a Lilith Fair coffee shop after party it was so subdued. When Paula Cole has more energy than you do it's a problem. Especially when it's at a festival w/200k people who were getting high energy sets from bands like No Doubt and Bush. Maybe if they had been at a small club I would have dug the way they were playing but at a giant festival on the main stage is was arse.
The giant arse water bottle fight that went down between people in the stands and those on the track was wild though I was lucky enough to be just far enough up the stands that bottles couldn't make it to us. Getting out of Texas Motor Speedway was terribad though. I've never been in traffic that bad and slow before or since.
Also, back in college I took a girl to go see Robert Earl Keen at the House of Blues in Dallas. He was her favorite singer so I thought hey, I'm looking like a great date now. For the opener we had a good time. We posted up in a great spot up front and were having fun talking to the people were met around us. Robert Earl comes on and right in the middle of his second song not 5 seconds after I handed her my beer a burly arse security guard grabs my date by the shoulder, drags her off to the side, and boots her from the building for drinking underage (she was 20 :-/ ).
She didn't get a don't do that again or else speech like most places or anything like that. The dude didn't even come after me even though he was standing right there when I handed her the beer. I asked him why the hell he kicked her out as I was leaving to go find her he looked at me and said the people standing behind us reported her for being too young to drink and that she was spilling booze all over their shoes and the floor. These were the same people we had been talking to during the opener.
Like an idiot I shouted that wasn't fricking possible since that was only the second beer I'd bought and I'd been holding them most of the night but once he got that mean Ima beat your arse b/c I can look I got out the door w/quickness. The only thing I can think of is those fricks behind us narced on us b/c they wanted to be up at the very front of the show.

the worst musical performance I’ve seen was blues traveler at red rocks a couple years ago. when they ran out of songs to play (badly) they just started alternating 15 minute solos among the band

No bad in concert experiences but post show wsp at the wharf 09(I think) peaking off a ten strip and saw a kid get his head bashed in with a tire iron by the nitrous mafia. Thought for sure I had seen someone die.

Summer of 1990 I went with my older brother and a friend to see The Allman Brothers at The Florida Theater in downtown Jacksonville (Similar to The Saenger).
I consumed probably one large vodka drink too many beforehand. I felt great no doubt but about the third song in I was standing at my seat and jamming, just really enjoying the music. Well the guy behind me started tapping me on the shoulder and telling me to sit down. Now this would not be my reaction today, but being in my mid-20’s and feeling no pain from the vodka I pretty much told him it’s a concert and to go frick himself. Next thing I know security is hauling me away and out the door I went. I figured at least the assholes could tell me to sit my arse down or get thrown out but nope. No warning.
And like I mentioned, third song in. As most people know, they’re known for their really long, 3+ hour shows. And this was before uber, before cellphone, and I had no cash in my pocket for a cab, so I had to just sit my arse right outside and wait. A little salt in the wound came during intermission when my brother and friend tapped on the window behind me and had a good chuckle. And oh yeah. Also missed a guest appearance by an 11-year-old Derek Trucks.
Pretty shitty.
Runner-up goes to the fricking Pixies a year and a half later at the legendary Milk Bar, also downtown Jax. Second song in they said something about the barricade up front having a problem and walked off stage. Not even five minutes later came an announcement that Pixies had left the building.
Sucked arse.
ETA: For absolute worst concert experiences though, I don’t think anybody can top the OP. Holy shite what a horrendous experience.

I have a few, Cheap Trick opened for RATT in Memphis in the early the mid 80's. They were terrible Robin Zander was drunk and messed up. 30 mins in he curses the crowd and leaves the stage, the band played for another 30 mins... and then left. Stephen Pearcy from Ratt came out and apologized with some strong language and promised the best show they had ever done. 3 hours of awesome rock. At that time they were as bit as Motley Crue.

Sound Garden opened for GNR... SOund Garden sucked and GNR waited until 1am to come on the stage and perform. About every 30 mins there was a live video feed from the dressing rooms with Rose, Slash, etc …
The show was great, my parents just about killed me when I got home.
a couple years ago, I went to the last motley crue tour. A friend got me so messed up, I don't remember the show much at all. Was told it was fantastic.
I remembered that night why I didn't drink like I used too. I haven't since.
at a Kiss Concert a few years back the show was ok for Geriatrics on Stilts. But after the show these three goons were beating the crap out of a kid, who was fairly messed up. My friend an I pulled a couple of kids off the dude and called 911. We stood there watching him and keeping the others at bay until help got there. I got sucker punched a time or too, but they weren't very tough to us older folks. I hate when azzholes mess up a fun night out.
I have so many memories, like having to carry a drunk friend across the mud island bridge in Memphis or risk going to jail. That sucked. Cop followed us the entire walk. and a friend rear ending my truck following another rock concert leaving mid south col in Memphis. Messed my truck up bad. We were all waisted as could be and this lady cop said, "I am going to ask you one question, make sure you think hard before answering, You Werent Drinking Alcohol … were you?" NO MAAM!
Then get back home.....

Cypress Hill at Bonnaroo in 2006. Started off ok...but they kept lighting up on stage. By the end, they were so high they could barely stand up unassisted. It was horrible.

I don't know, I saw them at the Saenger very early on and it was not a good show. The seats were pretty bad so that had a lot to do with it; the acoustics were bad where we were at and the vocals were 100% unintelligible (maybe just a REM feature). Our dates were not impressed with Stipe wearing a dress either and wanted to leave from the beginning. Hardly anybody knew who they were at that point. The show just seemed really flat; but people with better seats may have had a different perspective.

I went to this, kind of forgot about the time it took to get out of there. I do remember almost being run over by a couple of members of No Doubt on a golf cart

I saw something very similar after a sts9 show at the tabernacle about 10 years ago. They hit this guy in the face and then one of them full on kicked him in the face when he was on the ground. I seriously thought he was dead.
