As has been mentioned, the clear winner is Hayden Christiansen.
He was cast to play probably one the top ten most iconic characters in the history of cinema and he shite the bed so badly that when everything is quiet, if you listen carefully, you can still hear it drip onto the floor all these years later.

Some great ones. I think, considering the details:
1. Hayden Christiansen-huge role, big buildup and budget, lots to work with plot and line wise.
2. Sofia Coppola in GF3 - epic fail, but a lesser role and older flick.
3. Costner in Robin Hood. Some leeway because it's in that cheesy overdone 90s era, but still.
Mitch in Dazed was bad but it was a low budget movie with a cast of (then) nobodies. Much lower expectations than the others.

Halle Berry as Storm. Her acting in the X-Men movies screams "I'm only here for the paycheck".
Jennifer Garner as Electra. Probably the worst fit for a comic book character ever. Made worse by terrible writing and direction.

Bosceaux beat me to it. Dennis Quaid's performance in Great Balls of Fire was awful. It was like somebody told him that Jerry Lee Lewis was a cartoon character.
