I don't think it's even debatable that he did personally. The last line he said would be totally pointless if he didn't as well as Chuck looking surprised.
That doctor acts disappointed when at first it seems he lapsed back but clearly has a surprised reaction when he gives the live as a monster line.

Saw it today for second time
Somethings I did not notice but I should have when I watched it first time.
- In movie when they are interviewing patients and he is talking to woman who axed her husband. Everytime she mentions the missing Doctor they cut to Chuck.
- When he is talking to Dr. Cawley and Dr. Naehring and they mention the missing Doctor is cuts to Chuck
- When Teddy gets in car with the Warden, the warden treats him like the mental patient he is. He isn't talking to him like a Marshall (first time I saw this I thought it was pointless scene, now it makes sense)
- When Teddy gets back (after ride with Warden) and the Doctors and Patients are coming out of room they patients are scared of him. Because he beat the other prisoner up.
-The movie they told us what happened but I did not catch it first time. The missing mental healthy patient when they find her. She treats Teddy as that is her husband, she was playing role of Teddy's wife and Teddy himself by not accepting reality and creating a fake life.
I enjoyed the movie the first time watching it and did not catch many of the things I did second viewing. I am sure there was a few more that I missed again that if I ever watch it a third time I will notice.

Just got back from watching it. GREAT movie
I kinda figured everything out, but enjoyed the ride. I first knew there was something up when they first arrived to the island, and they asked for their guns, and the doc couldn't get the gun out.

Loved the movie. I perceived reality/identity within a reality/identity. I interpreted the end to mean that he choose to live without a brain rather than live in the reality of who he really was , an alcoholic who neglected his wife and didn't get help for her. He felt responsible for his childrens deaths and he killed his wife. He didn't want to live with the reality of who he was. HE CHOSE TO BE BRAIN DEAD. As evidence when he didn't turn around when the doctor yelled "Teddy". Which ment he was really living in his real identity and faking his responses to the doctor in the end. He didn't like "living as a monster" in his own mind not the crazy house. HIS Own Mind. The psychiatrist were there to assist him in his healing. Best show of year by far. If immerse yourself in his character it really makes you step back and questions your beliefs. Wild stuff!

ETA: After reading this thread, and realizing all of the little reveals...
-throwing up on the ferry ride
-the overly alert/hostile guards when they first get off the ferry
-"we're all on edge right now."
-"Chuck" not being able to get his holster off at the gate
-the other patients being scared of him
-Rachel's conversation in the cave, "They're ALL in on it."
-the warden talking to him the way he did
...they gave throughout the movie, I'm liking the movie more and more. I wish I was smart enough to pick up on them during the initial viewing.
Very cool. This movie's very Memento meets A Beautiful Mind.

I had to go see this for a second time. Since I knew what was going on already I was able to pay close attention to smaller things going on, for instance watching Chuck/Sheenan around Teddy/Andrew especially when Teddy would get riled up,Chuck would glimpse at Dr. Cawley as to ask if its ok to let Teddy go on.
But I have just 2 questions before I rule out he is 100% insane
1) How does it come about that he was able to hurt George Noyce, we all know that Noyce was in ward c, but were never told where Teddy/Andrew was, so how does he able to beat up Noyce, if Noyce is locked up there? I doubt Teddy would have that much interaction with any other patients if he was their most dangerous patient, and since hes living in his supposed own world wouldn't that make him that much more of a risk to hurt people? so why would he have interactions with anyone but his doctors and staff?
2) How do they get him to start his role-playing game? If he thinks he is Teddy the U.S. Marshal while already there, how do they convince him on the ferry? It seems from the way Dr. Cawley described it, he was always in the Teddy state, had a breakthrough, then relapsed correct? So how do they get him on the ferry to begin the game?

Thought they said Teddy was a patient in Ward C. I'm guessing he was kept around other patients in that ward. Looked like they had some patients in the same cell.

Really? Seems pretty understandable, the put him and Dr. Sheehan on the boat, send them to the other side, maybe even let him sleep the night in a room on the boat, then in the morning wake up and come out to where sheehan was standing

Great movie, saw it 2 weeks ago. However unlike yall, no offense, I'm not sitting there trying to guess what the twist is to ruin it for me. I was letting Leo, the main character, convince me he wasn't crazy.. Which is what I believe the directer/writer wants the whole time. I let the twist hit me pretty hard in the end and enjoyed it.
The way I interpreted the ending was that yes, he was crazy, yes they brought him out of it and showed him who he really was. And the "monster" quote was him expressing to ruffalo that he didn't want to live knowing the trauma he'd been through so he went through, he was choosing the easy way out.

No, it's called art. The ending you choose will end up saying more about you than it does about anything else. Which is what seperates good/great art from bad/average art

Haha! So I skimmed the first page of this thread, when it started, and for some reason I took it that this was the twist. So I go watch it last night thinking that they're all batshit and running the island. I figured Chuck was going to be the one sane one in the end and something was going to happen to him.
Then when Teddy met the "real" Rachel in the cave, I thought that was confirmation. It looked like the patients had turned on her and committed her.
So, thanks to a combo of Dan's post and my ADD, it was probably even a more interesting movie for me.

I thought it was going to more about the people on the island than Leo's character being crazy or not.
Good smart movie, but was expecting different. It was very cool how from the audience's perspective ti looked like BS until the evidence kept coming that he was crazy.
