Let's get some real nut jobs and not just the bull shite activists. Some of them kept it together enough to salvage a career while others never recovered.

Tom Cruise lifted the curtain a bit a few years ago, but he seems to have gotten things back under control.
It's probably also worth mentioning actors that lifted the veil enough on their awful personalities that it hurt their careers. Katherine Heigel comes to mind, as does Julia Roberts and others that are just nasty people whose movies I can't watch anymore.
