Jeremy London was so high during the making of Mallrats that Kevin Smith, who himself admits to always working stoned, didn't want to work with him anymore.

There's good stoned or fricked up in a set and then there's bad fricked up.
The Martin Sheen scene in Apocalypse Now is a perfect example of "bad" as isColin Farrell in Miami Vice.
They were rlegit screwed up, knew it, and needed help. They were in the total throes of addiction and alcoholism and all the demons they brought out in them.
Ferrell seriously doesn't even remember filming Miami Vice and got help shortly after.
The hotel scene basically is a real scene of a man at war with himself. The cuts are real. The breakdown is real.
Now Charlie on Wall Street or even Hopoer on Easy Rider. Those are example of fun fricked up. Those guys you could tell were just Ballin out.
I remember Oliver Stone saying how great Charlie could have been in both his movies of he actually cared and tried and wasn't on drugs all the time.
Hopper was so screwed up on Apocalypse Now that Brando wouldn't even film with him.

Mike Tyson when he was filming his cameo for The Hangover

Robert Downey, Jr was on drugs in basically every film he did in the 80’s. Weird Science, Back to School, etc., but he was next-level fricked up in Less Than Zero. I think he probably used the excuse of needing to be in character to stay lit the entire time.
Now that he is clean, you can tell how fricked up he was in his earlier films.

The entire cast and crew were stoned out of their minds during the filming of Caddyshack

Robert Shaw was drunk off his arse in the Indianapolis scene in "Jaws".
Richard Burton was drunk or stoned during "Taming of the Shrew" and "Who's Afraid of Virginia Wolff?"

I remember reading an interview where Jack Nicholson said in a scene for Easy Rider where they were smoking weed they got super stoned before and were smoking real weed during the shooting of the scene.

Shannon Hoon was tripping on acid during the filming of the video for No Rain if that counts.

JCVD was so coked out during his prime in filming, that producers had to hire a "wrangler" to follow him and keep him on track in filming
Read that during the filming of Street Fighter, JCVD was so bad, they never thought the movie would get finished. He couldn't remember lines and couldn't stand up straight

I'm certain it happens ALL THE TIME. Hell, I was just watching a documentary on Monty Python and it was widely discussed that if they wanted to get anything done with Graham Chapman they had to do it before noon (if they could at all) because he would get bombed on gin and/or vodka every day. Apparently filming Holy Grail was a complete chore.

Spielberg new about Tom Seizmore having drug issues and required that he had weekly drug tests during filming of “Saving Private Ryan”.

This was the first one that came to mind.

Have to think Corey Haim belongs on this list.

Everyone except for Ted Knight. I read that he was royally pissed off at the unprofessionalism of his co-stars.
I assume that a good portion of the Dazed and Confused cast was on something.

Why would they? If actors are to screwed up to show up or can't perform their lines it cost millions.
Record labels and concert promoters are the same.I went to numerous concerts in the late 70's and 80's where performers were unbelievably messed up and some didn't go on stage for 2 hours up til after the show started if at all.It happened at an Aerosmith concert I attended and tickets had be refunded.Same with the Allman Brothers
It rarely happens now because there's way too much liability involved and social media also plays an impact.
