Final Update. I do indeed have a prolactin secreting tumor on my pituitary. It's less than 1 millimeter which is why it didn't show up on my MRI. The fix is just to get on TRT, and watch the prolactin levels, as well as an MRI once a year to make sure its not growing in size.

TLDR: Pay attention to your body. If you know something is wrong, get it checked out, and push to get answers.

so what is your current regimen and what does it cost?

so my regiment is

70mg twice weekly of testosterone cypionate. I can do intra-musclular shots in the butt, quads, shoulders, or ventriglute or i can do sub q in the stomach fat.

I give myself the shot and because i do not have estrogen problems, i take no Aromitise inhibitor and i do not take hcg since i am fixed and want no more kids and could care less about the size of my balls. hell smaller they are, bigger the dick looks

I see a urologist at baylor medical. I see him once a year and then i do lab test here locally in between visits.

as far as cost, mine is covered by insurance but it is actually cheaper to use goodrx. i get the 10ml vials and just check the website to see who is cheapest, usually it is CVS for me. You can get a single vial for like $36, but if you get 3 you can get for like $90 and that is like 42 weeks worth so looking at less than 150 a year even including the needles. I get my needles from gpz services.

he wants me to get on growth hormone peptides as my igf levels are always low, but really dont want to do it as its expensive.

i dont mess with the things like anavar, nandrolone or some of the others the "hormone" clinics prescribe.

gonna piggyback here

early 40's

total test - 321 ng/dl
sex horm binding globulin - 18.2 nmol/l
testosterone bioavail, calc - 200.8 ng/dl
testosterone calc free - 85.7 pg/ml
testosterone % free calc - 2.7%
tsh third generation 1.67 uiu/ml

everything was within range so my primary care didnt say anything. should I be trying to do something or should I be good with being on the border of being low?

so for total T- you are within range but at about the 10.8 percentile which isnt very good for early 40s

shgb- is high

free test- about in the bottom 10 percentile on this also

here is a good slide show on hot to do treatments, this was presented at an endo conference LINK

So things your doctor needs to look at outside of libido and energy

1) what is your body fat percentage(not just BMI)-so need to get your body fat % somewhere below 20%, closer to 15% if possible, and recheck test using morning sample and to make sure the test is accurately done by the cdc guidelines. Should get atleast 3 test in the morning after fasting 8+ hours

2) what is your current exercise- If oyu are currently not doing a resistance training program, you need to start. If you are doing excessive slow long range cardio, you need to cut back on that.

3) sleep patterns- if you currently are not getting 7 hours minimum sleep every night, need to start. Use melotonin to get into deeper sleep. if you have possibility of sleep apnea get that checked first. also have IGF levels checked.

4) test your BP, hemotocrit, calcium score, psa, hdl/ldl- this is important because it gives baselines and also allows you to customize your diet to get these in order

5) diet- you need to look at your diet closely. Cut out processed crap. Cut back on veggies some and fruit some and focus on getting better quality fats. Notice i did not say cut out veggies, fruits or even carbs like rice or potatoes, just cut them back and start eating a diet that is high in protein, medium fat and lower carb.

the above are steps you should take before doing anything as far as going on therapy. Allows you to get baselines, get exercise/diet in order and bodyfat % in order. This will allow you to know if its a physical issue or that it was more of a lifestyle issue.

Technically within range but scraping the very bottom of the range. Very close to where I was before I started TRT.

thats a question of genetics. true trt doesnt put you outside the normal range so shouldnt unless you are predisposed to it.

pbf according to my dieticians fancy scale is 24.5. according to her analysis I need to gain about 13 lbs of muscle. I am currently skinny fat.

2) what is your current exercise- If oyu are currently not doing a resistance training program, you need to start. If you are doing excessive slow long range cardio, you need to cut back on that.

no exercise currently. any recommendations? my problem is I just dont have the time or even know where to start. Im always on the road also and schedule is crazy.

3) sleep patterns- if you currently are not getting 7 hours minimum sleep every night, need to start. Use melotonin to get into deeper sleep. if you have possibility of sleep apnea get that checked first. also have IGF levels checked.

I usually sleep pretty well

4) test your BP, hemotocrit, calcium score, psa, hdl/ldl- this is important because it gives baselines and also allows you to customize your diet to get these in order

if by bp you mean blood pressure, it is usually 120/80

hematocrit - 43.2 %
calcium - 10.1 mg/dl
I dont see a psa unless it is called something else
hdl - 48
ldl - 146 which was high and she said that she would see where it fell next year because I have been cutting carbs to help control blood sugar and that may contribute to higher ldl

5) diet- you need to look at your diet closely. Cut out processed crap. Cut back on veggies some and fruit some and focus on getting better quality fats. Notice i did not say cut out veggies, fruits or even carbs like rice or potatoes, just cut them back and start eating a diet that is high in protein, medium fat and lower carb.

about 6 months ago I was close to having prediabetes so I cut all carbs out of my diet and lost 15-20 lbs. I was concerned because I wasnt much overweight and I started going to the dietician to help me better understand how to eat better to not get diabetes. I think I am still not eating quite as much as I should because I am not a big eater and I usually only eat 2 meals a day. Now that I am not eating tons of junk or fast food, I feel like I am not getting enough calories. I do eat a good bit of meat so I feel like I am getting sufficient protein but maybe not enough calories. I thik I am pretty much doing what you say, although maybe not as much food overall as I should be eating. My blood sugar a1c has improved and got me a good bit away from the prediabetes area.

Still a work in progress. Like I said, she wants me to gain back 13 lbs of muscle but I have never worked out or been a muscular dude. She wanted me to check the test because she was concerned if my test was slow I wouldnt be able to gain muscle.

see above

yea not sure on your weight but BF% is higher than it should and getting this lower and building muscl should help.

ok so sounds like you are making some changes with diet. I would try and get 3 meals in a day with 8-12oz of meat in each or replace one with 4 eggs and 2 strips of bacon

main thing is got to get you doing some form of resistance training and walking more.

when you say you are always on the road....do you mean you are out of town staying in hotels a couple nights a week or what?

when you say you dont have time....do you mean you cant do an hour or 75 min 3 times a week? what about twice weekly?

yea different calcium score. I was talking abotu a score for calcification of the heart arteries and psa is prostate score.

but i think if we get you lifting and walking, you will be fine and your score will go up. add in better diet and it will be even better.

Just had my test levels checked.

Overall: 370 ng/dl
Free: 6.9 pg/ml

Overall is lower in the range but free is low out of range. Doctor asked if I was interested in TRT.

Cortisol levels were 12.6 ug/dl early AM that day.

Diet is fair. Not great or bad.

Sleep is erratic. Neck has been bothering me recently and affecting my sleep.

51 and I don’t drink much. Had a handful of beers at the tailgate last week, but I rarely have more than 1 if I have a drink, which is about 1x per week if that.

I’d say my overall stress is low.

Currently 5-9 198 around 16-17% bf.

All other bloodwork was great.

Nearly every day..

Right now, Bench is sitting around 275, Squat around 375 and deadlift around 405.

My DL has always lagged behind.
