Tell me what gaming system to buy?

I haven’t played anything since the PS2 back in late 90s/early 2000s and mostly just sports (NCAA, Madden, etc). I’d like to casually start again, but completely lost.
X box, PS, any other systems? What’s the OT recommend?

Get the PS5. The new NCAA will be coming out this summer. They haven't made one in years.

Do people still buy physical games or are they linked to the internet and downloaded from the machine?

Whichever is your preference. But some games are download only. Most all the big titles have physical copies still, but I'm sure that will be phased out over the next several years.

Xbox Series X for Gamepass and you will have access to a bunch of EA Sports titles as well as a ton of other games. Its really the best option for casuals and especially since you've missed out on several generations you can enjoy some of the great older titles on there.

Even if there is a physical copy of the game, most if not all games still require patches to be downloaded from online.
Switch games in particular will have a physical cart but you still have to download a gigantic game online in order to play anything or have you buy a case with just a download code inside.

Should be right up your alley:

Obviously you like sports, but what else are you into. May come down to the exclusives for each system.
