The best I've ever had is the Antoine's cocktail sauce in the recipe collection. It's a 2 step process, but well worth the effort.
Antoine's Cocktail Sauce
Vinaigrette Sauce
1/2 tsp salt
1/3 cup vinegar
1/4 tsp finely ground white pepper
1 cup olive oil
1/2 tsp dry powdered mustard
Put all ingredients into a bottle and shake to mix.
Cocktail Sauce
4 Tbs horseradish
1/2 cup Vinaigrette Sauce (recipe above)
3 Tbs Worcestershire sauce
2 cups ketchup
1 Tbs Tabasco sauce
1/4 cup lemon juice
Blend all ingredients together and chill. Makes 3 cups.

The bartender at Felix's downtown told me he wanted to make me his version of cocktail sauce for the dozen raw I ordered so I said sure.
He came out with it and it was delicious, all the traditional ingredients(horseradish, ketchup, lemon, hot sauce, worchestershire) but there was something else.
Told me if I guessed it he would tell me what it was.
It was liquid smoke.
Wasn't a lot. Very subtle. Very good.
Don't think I would want it every time I ate cocktail sauce, but it really complimented the dozen raw I ate.

This. Better if you can find fresh horseradish and grate it yourself

Ketchup
Chili sauce
Horse radish
Buffalo hot sauce
Worcestershire
Lime juice
Black pepper
I also like to add a little chopped up cilantro which pairs well with lime juice and freshens it up a bit.
