I'm a fit, trim, active 50 y/o who just found out i have genetic condition that causes very high LPa levels (all other levels great, no other risk factors), then comes the Calcium test showing 0 for all but 1 arteries, but that 1 is the widowmaker with a 120 score. I'm in search of a cardiologist who will be aggressive in approaching my treatment with my specific conditions.

there is no treatment for high LPa. There is a clinical trial drug out there - so you might try to get on the trial.

Some say taking Niacin might help.

It's an inherited condition. I have low LDL/Cholesterol and high LPa as well as a bad calcium score.

You need to get a Cleerly scan, pronto. It will give you WAY more workable info than a calcium scan.

I have high lpa as well at 45. My brother has it as well. Still 0 on calcium fortunately. Looks like a lot of stuff coming down the pipe in the next year or two.

Dr has ordered an ultrasound stress test. Also a pcks9 inhibitor which lowers APOb and can reduce LPa a little bit- 30% or so

Get the Cleerly so you know exactly what you're dealing with.

Assuming your other CAD risk factors are ideal (bp etc), whichever doctor will go through the legwork to get you on a PCSK9 since insurance may be reluctant to pay. That could be your pcp. That’s the best you’ve got for elevated lpa for now until new drugs are brought to market

Not I sir, but the one drawback I see is the radiation exposure which is fairly significant. I’m doing the stress test echocardiogram tomorrow which isn’t as sensitive as Cleery but, if it is good, then I’ll have a real question of should I ask for it or not. I appreciate your input

Yeah, fair. I think with the newer ones, you can get it as low as 3-5.

I checked their map - looks like the only location in the state is in Lake Charles.

For the north LA folks, there is a location in Shreveport that offers the Cleerly test: Integrative Partners offers Cleerly cardiovascular testing.

There is also HeartFlow: I am copying and pasting.

quote:

HeartFlow analysis, a non-invasive tool for coronary artery disease, is available in Louisiana primarily through specialized cardiology centers. Key locations include the Cardiovascular Institute of the South (CIS) (Houma, Zachary, Lafayette), Our Lady of the Lake Regional Medical Center (Baton Rouge), and LCMC Health/West Jefferson Medical Center (Marrero).
Key Louisiana Locations for HeartFlow Scans:

Houma: Cardiovascular Institute of the South (CIS), recognized as the first in the state to adopt the technology.

Baton Rouge:
Our Lady of the Lake Regional Medical Center (Heart & Vascular Institute).
Louisiana Cardiology Associates.
Lake Imaging Center.

Lafayette:
Our Lady of Lourdes Heart Hospital.
Cardiovascular Institute of the South (Lafayette on Heymann).

Metairie/Marrero:
East Jefferson General Hospital (Cardiac Imaging).
West Jefferson Medical Center (Heart Clinic of Louisiana).

Zachary: Cardiovascular Institute of the South.

I can't recommend a specific Dr but look for a Cardiologist who specializes in the treatment of high LPa levels.

Here are a few in the BR area that specialize in the treatment of familial hypercholesterolemia and LPa.

quote:

Dr. Garland G. Green (Baton Rouge): An interventional cardiologist recognized for expertise in lipid disorders.

Dr. Denzil Moraes (Baton Rouge): Distinguished provider for Familial Combined Hyperlipidemia.

Dr. Thomas Lemay (Baton Rouge): Distinguished provider for Familial Combined Hyperlipidemia.

Update: Stress Echocardiogram was great. Got up to 98% max heart rate, had them check my VO2Max and it was also great. They looked at the LAD and found nothing that would indicate any significant blockage, no arterial stenosis (which is another area that LPa affects)....so now i'm down to that one risk factor, high LPa. FWIW, i started taking high flavanol cocoa (Cocoa Via) about 4 weeks ago and, while my BP wasn't bad, it has dropped by about 5 points (avg 132/78 down to 127/75). So something to consider for any of you wanting a supplement. We shall see if the insurance will approve a PCKS9 Inhibitor, which could lower LPa and drop my APO B do further decrease risk. Thanks again for all the support!
