Looking at a plate left on a table in a restaurant

There is a sandwich that wasn’t touched, a fork on the edge of the plate, and some other items.
The person has been gone about 30 minutes.
It represents a moment in that person’s life that has come and gone. The fork was very important to the person but now it sits abandoned, and obsolete.
The sandwich is not touched and I imagine it is waiting for someone to return.

There is a much bigger mystery going on here. The sandwich situation is obvious. The guy had an important phone call and stepped outside. He was then mugged and shot. At the emergency room he required emergency surgery. But the surgeon came in, took a look at him, and said, "I can't operate on this man, he's my son." The thing is, the dude's dad died years ago! frick the sandwich, who in the hell is this surgeon?
