my wife is a licensed therapist (LMFT), and I hear all sort of crazy stuff that she has a heard about during sessions. No names or pictures, but the details are hard enough to hear sometimes. I've heard many a story about handsy uncles/stepdads, physically and mentally abusive parents, rape, violent crime witnesses, etc. I'm pretty sure she just has to tell somebody some of the things she has heard because they're so messed up and dont want to internalize all of that.

I imagine most people take this stuff seriously, not just because of professional obligation but just as general decency. Also, for the vast majority of us (at least with lawyers) it's not that interesting to anyone else but other lawyers, and even for most of them none of us care about the details.

None, I'd imagine. I'm pharmacy end but HIPAA (and fraud/bribery/corruption) is basically the only thing anyone takes seriously

I have some friends who are first responders. They've told some of the stuff they've seen, mostly mild. I know they've seen worse but I don't really want to know about it.

A friend of mine is criminal defense. The stories about the alleged crimes are interesting, but when he gets into the legal jargon, it gets real boring quickly.

I don’t think that doctors usually go around telling their friends stories with people’s names attached.
And there’s all sorts of rules about what is potentially an identifier under HIPAA (like for instance ZIP Code can’t be quoted if it has less than 20,000 people in it), but telling a general story without identifiers isn’t illegal.

Not many.
We will discuss or share stories about hard or assholes patients, but never use identifying information.
Hippa is taken seriously in all the facilities I work with.
For instance, I've heard about the weird stuff people put in their arse, but I would never be able to identify somebody that did it.
You should be safe from whatever your worried people will find out about you.

It's usually the unskilled front desk receptionist who does the gossiping.
Healthcare professionals in my family take that very seriously, and I'd never jeopardize their livelihood by ragging them to tell me names.

I think most places take it pretty seriously. Some organizations have taken a zero tolerance policy and that can help staff understand better. Most, if not all organizations should require some form of privacy training at hire and annually after that. Some training is better than others. I've done hundreds (if not more) of privacy investigations at several health systems, and I've seen the worst of the worst. Love triangles, small town gossip, department gossip, curiosity, willful disregard, and social media posts. In 15 years of investigations, what I've encountered is that emergency departments seems to have the biggest issues but I've seen it all over the place. I've seen staff at all levels (janitors, coding, billing, all kinds of clinical staff) be fired for privacy violations. Some days I've felt like Jerry Springer talking to these people about what they did and why they did it. I've also, seen the govt fine organizations big money for some cases.
