USSF sanctioned youth games mandate them, as does HS (NFHS), at 85 degrees or hotter.

Back in the old days, I could have sworn catching a rare soccer game on TV, and commercials would break in during the match as it played. Then it was a sigh of relief that the 94 WC agreed not to share this pattern. I could be remembering wrong but I did find this article.

They have "hydration breaks" because if they called them "pause the game so we can sell some commercials breaks" it would be too obvious. Player safety is the last thing on their minds.
