Because I read it as "Almost failed and (in failing it did) cost dozens of American lives".
These people are *supposed* to have a command of the English language. They are either incompetent, or intentionally writing this to give the impression of lost lives. Or both.

Amazing how words can be used in a headline, where they force, you the reader, to read between the lines to get the real truth. If the average reader was not careful, they would have missed the word 'almost' and would go around telling people, "Oh my god! We lost dozens of Americans!"
This is another prime example of why you truly cannot trust the media! They try to distort the truth to appease their base of followers.
