It’s likely just a means of indicating that’s a direct quote from the interviewee; IOW, Gaines referred to his junk as male genitalia.

Sure, but they quote her directly in the next line. Either say male genitalia, or say genitalia without mentioning sex. By putting it in quotes it calls into question the claim...at least as far as I read that.
