The ears are definitely different. I'm not a conspiracy nut but those are not the same ears.
ETA- I didn't just compare the OP pic. I looked up photos on google img search and looked at dozens, and the guy on the right in OP does not have the same ears.
I'm more inclined to believe it's a deep fake photo than an actual body double.
