Dude lost a ton of weight.
Also, photographs can be deceiving. Different focal lengths on lenses and the distance to subject can make your head appear misshapen. Lighting too makes a difference. You are comparing a professional headshot to an cellphone picture.
Also, you would think someone would have gotten him some newer clothes that fit to get out the hospital. He looks like a hobo and not the Junior Senator of Pennsylvania.
Despite losing all that weight, am I the only one who thinks his man boobs are bigger than hers?

It is. It was first published when he left the hospital following his stroke. The point remains however that those two individuals are NOT the same person.

ETA- I didn't just compare the OP pic. I looked up photos on google img search and looked at dozens, and the guy on the right in OP does not have the same ears.
I'm more inclined to believe it's a deep fake photo than an actual body double.
The end result is the same. One of those two isn't John Fetterman.

Rumor has it that Fetterman is in such dire condition that he will require a brain transplant from a deceased Democrat. Unfortunately none of the deceased Democrats available appeared to have a functioning brain either. Now they have begun conducting a survey of the living ones in search of a potential donor and still have thus far been unable to find one with a functioning brain.
