Is their nose hanging out of their mask? Did they wear it around their chin or ear before putting it on? Are they in a sterile environment?

Define “effective.”
Two “two ply” paper towels are more effective than most masks in one study. Mask have a limited effectiveness...but they still help to slow the spread (not stop it).
However, those studies haven’t been based on a truly airborne virus (not “droplets”) . If that is how Wuhan flu is spread (which has been alluded to by Dr. F), then as I understand it, only an N95 that is properly fitted would be appropriate.
I wear a mask in public buildings with large crowds. For the most part though, it seems to be simply a placebo to minimize public anxiety.
The virus is still going to spread.
