The entire thing seems fricked up. Tower was talking to the instructor, seems like too many people involved for such a critical juncture. Its obvious the actual pilot never saw the plane.

Ive flown small planes (172s and 150s) and had much better visual instructions than these folks appeared to have. I'm not sure everyone was on the same page.

If one flight was IFR, then there ia culpability for ATC. They became that flights "vision' and are responsible for separation between aircraft.

Didn’t ATC inform the helicopter that they were to high/off course and told them to fly behind the plane?

And you are totally correct about solo fault accidents. I think people don’t realize that (not saying the poster you replied to is insinuating that) because a lot of accidents are classified as ‘pilot error’. The media won’t go into details as to why the pilot made the error, was it lack of training, bad information (like from ATC or equipment).

I think the blame in this crash will go more towards the pilot of the helicopter. Perhaps ATC didn’t notice the helicopter sooner. I haven’t been able to really get into this crash like I’d like to.

Yeah I agree, the point is rarely a mishap is a single reason. The 100% blah blah is as someone else noted a very simplistic comment.

After watching these clips, it certainly seems understandable why some people may come to that conclusion.

As for me, I still am holding to the assumption that this was a horrendous accident compounded by a series of critical errors both in and outside the helicopter.

Whatever the case may be, the entire CoC that signed off on this training mission should be held accountable and face some measure of impactful repercussions.

It’s simplistic because there’s a simple reason for the crash. The helicopter was too high.
