Bet that there were contributing factors? Why would I do that? I just listed all of the contributing factors.
And once a pilot claims to have visual sighting of conflicting traffic and the controller says “maintain visual separation” then it’s the pilots sole responsibility to not crash into the other airplane.
So yeah, there were other “contributing factors” but it’s 99.5% on the Blackhawk crew.

You may very well be correct, but in my experience, there have always been a number of chains in the aviation mishap, any of which had been stopped could have prevented the mishap.

None of that matters if the Blackhawk flies at the right altitude though. It was piss poor airmanship by the Blackhawk crew.
If-if-if-if the helicopter woulda-coulda-shoulda been at 200 feet, a passenger jet would have flown within feet above it, which btw is apparently what had happened in the past .... and apparently in this instance it would have been treated as "business as usual" there. You think tolerance of a series of near misses involving the nation's busiest runway is acceptable?
