Course work is a small portion of a doctoral program.
It takes years of independent study and research. At the end you have an oral exam where you prove that you are now not only knowledgeable in your field, but you actually understand your research at a level beyond your advisors.

Getting a phd is difficult for many reasons, in general, particularly when compared to the accomplishments of the average Joe. However, in the context of being a college student, it is much more about determination, time, and money than anything else. I agree with that part.

For whatever it's worth, I disagree with the OP's assessment of the student. I'm just speaking in regards to the ACT, which is actually an unknown variable for that.

Stuff like Algebra and Geometry? I would think by the time you're a junior in high school. And certainly by the time you're a senior.
I took Algebra in 9th and Geometry in 10th.
Science is just reading and interpreting graphs and understanding the scientific method. That's all covered before you're a junior.
English is grammar (Elementary and junior high) and rhetorical skills (looking at passages and answering questions).

I get your sentiment for some inner city school where the kids are struggling etc., but that just doesn't hold water for a student with a 4.0 at a high school in any state. He graduated and took all of the basic classes, at least, that are required to do so. He made all As his entire career. If he doesn't have a moderately high ACT score, he has no excuses.
