A lot like New Orleans and south Louisiana. All three of these places south Louisiana, Italy and Spain are all made up of a people that like to socialize, are very affectionate towards one another and like to party and have festivals. Perfect scenario for transmission of a contagious disease and that's why these areas seem to be the hardest hit.

Testing. Death. Autopsy.
First, testing has been sorely lacking and the numbers in the US are almost meaningless.
Secondly, Death. Cause of death with no prior testing will be "Other". IOW, natural causes. Pneumonia. Whatever.
Lastly, testing after death. Not only will autopsy be delayed if numbers start to pile but there is absolutely no way our local/state/national government wants the cause of death to be associated with misdiagnosis and subsequently, open for litigation.

Well, the reasons being thrown out like multi-generational homes, general age and health of the populace and health care really doesn't fit that well with Italy. The areas being devastated are the wealthiest Northern regions where all those factors are much, much better than in the south which is hardly worse than elsewhere in Europe.
That certain places where people have much more socially interactive lifestyles-- if there were many carriers in that area -- have had greater explosions is certainly holding true though.
However, that still means the south of Italy, for example, seemed to avoid many "carriers" early on before the various social distancing and lock down measures started being implemented 2-3 weeks ago.
On a side note, there are many experts saying that NY area would need to have had many more cases of this late in January for the spread to be as acute as it is right now.
