Dr. W. Junius at Crescent City Orthopedics did mine in 2013 and I've not had any issues with it since then.
Be forewarned, if you do need surgery, it is a pretty long recovery process between surgery and getting the PT needed to get it right-----and I'm talking months, not weeks. I had mine done in October of 13 and had my final PT sessions in March of 14.

Dr. Wax at Jefferson Orthopedic or Dr. W. Junius. Both very solid.

Buddy Savoie at Tulane is the answer and this comes from someone who had Junius did his without issue. The only reason Savoie didn't is because he wasn't in network with my plan at the time
