This is absolutely on the engineers.
Hate to say it but Heaslip was in over his head. No experience with this type of building, office full of recent college grads, pressured by ownership.

These contractors do not have their own engineers who are checking up on the project? Not saying they weren't at fault but their job wasn't exactly made all that easy by the demands of the owners.

On a lot of projects this role is fulfilled by a third parrty testing lab/engineering company like Terracon
It’s pretty clear from all accounts this project was cut to the bone cost wise so i doubt that happened
