Traditional?

Unconventional?

Expected?

Not expected?

Family or friends help with the planning?

Strangers?

Co-workers were witnesses?

Ask for parents permission?

Situations are different for every couple and some people don't.

I did, but that's me.

Gotta ask the dad for sure. I surprised mine at work the same day I signed the papers on our house.

I dated my wife for 7 years before asking for her hand in marriage.

In hindsight, I should have waited 20 years.

Went to Oktoberfest, then took train to Prague and on last day of the trip proposed on the Charles bridge. It was a surprise and she didn't expect it b/c it was at the end of the trip (they are trained to expect engagements at the beginning of vacations).
I lucked out and got a young Spanish couple in the old I'll take a pic of you guys if you take one of us so I have a picture of it.

Yes I did ask for the parents permission. They lived in Dallas so I had to Skype.

I think the key, more than anything is understanding that it will be a thing she'll want to remember, so find a place or landmark (doesn't have to be fancy or even one that holds significance for you two currently).

At home. For reasons I won't go into, it involved a fair amount of planning and was a huge surprise for her. Had a lot of stuff planned that day (winery, nice dinner, etc.) but due to some trickeration I still got her pretty good.

I was apprehensive about doing a proposal there, but I couldn't have pulled it off like that elsewhere and she's not a fan of cheesier proposals anyway

This lucky fella just did.

Poor co-worker didn't even have time to put away the toilet plunger.

Told my wife I needed to borrow her camera to take pictures of a client's property. Took a bunch of pictures of some random mansion, then a picture of a sign asking if she wanted to marry me.

Went home, told her to scroll through and look at how ridiculous the house was
