Your friend needs to go to the nearest police station and file a report that states his current girlfriend is not emotionally stable and has a history of suicide attempts. Let them know that her brother committed suicide this past summer and that your intentions are to break up with her, but want her to have the proper help and see what they say. They may turn you away, or they may offer assistance. Either way, he needs to end it with her and find some sort of way to get her into a mental health clinic.
It's a lose/lose for your friend. He either stays and is miserable and does something to set her off one day that could put his life in danger, or he ends it and she ends her life putting her life on his shoulders.

A few options.
1.Call one of her family members, tell them the situation and that you think someone needs to be there when she gets the news. Have them text you when they arrive and this is one time its okay to break up via text so send her a text, let her know she can call you if she needs to, but at that point its in the hands of the family member.
2. Tell her you have been fighting this as long as you had feelings & attractions to other people, but you are tired of fighting it and that you are gay.
3. Do something really fricking stupid that would make her break up with you.
4. Just cut off all communication with her. Take all the vacation time you have available & get out of dodge for that long. When you come back dont call her, go over, etc. If she comes over, lock doors, hide where she cant see you, etc and if she comes to your work, get a restraining order on her and go from there.

Your friend is 40. I imagine he has a job, growing corn or whatever the hell y'all do up there. A man with a job can find a decent woman.

YES! He and I had very similar childhoods (shitty) but my life is pretty awesome and his is all sorts of suck.
