It would depend on your definition of terrible. I've gotten offers out of interviews that I didn't think were great, but I wouldn't have called them terrible.

I will tell you this, every interview I went to in which I got the job, I was not dressed in traditional interview clothes (suit). And there have been interviews I thought went really well.. And never heard back from them... If that helps.

I got a job where I was absolutely sure the interviewer didn't like me or at least picked up on the fact that I didn't feel too good about him. I was shocked to be offered the job. There was a very palpable awkwardness and vague hostility there in the initial interview. I needed work so I accepted and managed to outlast him too. It was a great day when he quit. Win!

I interviewed with a company a few months ago and the first part went great.
Just so happened the VP of NA Sales was in town and interviewed me for the second half.
It was terrible and I'm pretty sure I blacked out at one point. Might have drooled a little.
Got offered a couple weeks later.
I ended up taking the job but only worked there a month. Too corporate for me. Resigned the day the VP came back in town.

I've always left an interview feeling that I bombed it. I've never been out of work, so I guess the answer to your question is all of them.

Of course it did. They interview people every day and you interview semi-occasionally. Their comfort level would exceed yours by miles.
Next, you go into the negotiation phase. Expect a lowball offer and go from there.
