The show was the modern version of a circus freak show.

I’ve scanned a patient for cardiac clearance who was potentially going on this show a few years ago. The mental aspect of how they use food to cope with other stressors is very interesting. There’s so much going on with these patients and many of them have enablers who won’t do the difficult things it requires to help their loved ones. The patient was very nice and her attitude was one of desperation and it seemed she knew she would die if she continued to live the way she was. There’s no excuse for being 400 pounds overweight but it did give me some perspective on how it can spiral out of control. These patients need the help of a therapist just as much as an MD to really get to the source and know what triggers and behaviors to avoid along with the benefit of actually talking to someone who can actually help them if they commit to being helped.
There is absolutely a mental health component to being morbidly obese -- whether that obscene volume of food is consumed as a crutch, or because of addiction, people of this size need mental health professionals.
On that show, therapy is usually a component of the treatment and the ones who are open to the therapy and assimilate what the therapist says are usually the ones who do better in losing the weight.
It's just all around a sad situation.
