What, exactly, did you ask your endocrinologist to check? I already have one for managing a thyroid disorder. Figured out with recent bloodwork that my testosterone could use a boost as it was sitting at 261.
I’m in good health, not overweight, don’t smoke, don’t drink, sexual desire has never been a problem and getting it up never has been, either. Considering TRT, but the side effects concern me. I just don’t see a “natural” route here given that there isn’t a lot of change to make lifestyle-wise. And from what I’ve read, all of those things you can do account for small gains. Nothing significant.

Mine also on cusp of low range around 250 (mid 40s). However, I’ve always been in pretty good shape. 6’ 177 lbs with great energy, sleeping habits, and sex drive. Lift, play sports, etc.
I’ve tried a few ‘natural’ things over past year like Ashwaganda, cold plunges, staying away from foods like bananas that are known to hurt T. I’ll find out at next physical if it’s helped at all. Will consider treatment if I ever show symptoms or maybe when I get into mid 50s.

What side effects are we talking about here? The lady I see keeps her mens levels around 1200. Some anti-aging clinics or hormone clinics will have you around 1500 depending on how you feel.
The side effects of testosterone and steroids in general, are vastly overblown. When something bad happens, its usually from a pro BB that is running 2000-3000mgs+ a week and using other types of drugs on top of it

Not a Doc nor a medical professional.
I deal will low-T as most men over the age of 50.
Lose weight to YOUR ideal weight. The traditional BMI is not that accurate.
I am 5'10" and according to the traditional BMI, I should be around160-175 lbs. If I got down to that range, I would look like a walking skeleton.
They now say the 'Waist-to-hip ratio' is the superior indicator of ones ideal weight.
Anywho, losing 25-30 pounds would definitely be a huge help.
Think of it this way. You are carrying around an EXTRA 25-30 lb sack of potato's every step you take...up the stairs, down the hall, at the job, out in your yard working...
Look at your diet...IDK how good yours is but if you are consuming lots of sugar, sodas, processed foods...that's not helping.
I have read that Pomegranate juice is very helpful.
There are tons of advice on what foods to eat and not to eat among exercise too.
Cliff notes: lose weight to your ideal weight & eat better.

400 mg of test is a super low dose. You’re not going to get jacked on 400 mg. IFBB pros are all using at least 2-3 grams of Test alone, not counting the 6 other drugs they’re stacking it with.

Probably a better discussion for the H&F board but I agree with you that side effects if used responsibly are largely over blown. But I do question the value of men in their 40s and up using TRT to keep their levels well over 1000. AT what point does it turn into more of a PED than true test replacement and does that distinction even matter?

You're out of shape. Start there by working out. Generally every 10 lb you lose will also lower your BP by 10 pts.

Again, bro, I played sports too. I’ve been lifting pretty heavy for at least the past 10 years. I’m your height. I weighed 183 lbs yesterday morning, and I could still stand to lose around 10 lbs.

Shouldn't matter IMO. As long as you feel good and bloodwork comes back clean.
The value would be you are less likely to fall and break bones if you carry more muscle mass. There are also some disease states that have low testosterone as a contributing factor, like Alzheimer's.

Yes I would. Believe me, I know. Several years ago, I lost a good amount of weight. I got down to about 175 lbs.
Folks thought I was on meth or crack or they thought I was sick and going through chemo.

The old BMI index is way outdated (and very inaccurate and can be deceptive) developed in 1972 by one Dr Ancel Keys.
Dr Keys index was largely based on the work of one Mr. Adolphe Quetelet, which was created inthe middle 1800s. Quetelet was not even a medical professional! He was an mathematician, astronomer, statistician, and sociologist
No doubt Dr Keys was a good man and well meaning. However, he got several things in relation to nutrition wrong. Mainly on the idea of saturated fat and heart disease.
The old BMI index does not take into account many measurements such as muscle mass, age, bone density, and body frame.
BTW, I have consulted with my doctor and we agree my ideal weight should be 190lb range.

We don't really know because we don't know his muscle mass, body frame, bone density...but yes I would agree the OP is overweight and needs to lose weight, with a healthy diet.

Yep, I know. Right now I am a portly 225lbs. My goal is the 190lb range.
Every person is different with diverse body structure, make up, muscle mass...

The lowest I've weighed in 15 years is 237lbs.
I have a picture of me turned and holding a bottle for display, you can count my ribs through my tshirt
At 290lbs I did water testing and was 21.6%. So, fat. But half of what BMI assumes.
Outliers exist.
