I've heard that his stomach is supposed to be the joke of the picture. They had some sort of white trash themed party and the lady in the picture is actually pregnant and couldn't button her shorts. He is too fat for his pants and couldn't button them, so since they both could not button their pants, they had their picture made together.
Heard that on a radio show.
