Sister is fair game. Ex depends on how close the friend and how long the relationship/breakup was, but usually it's a no go.

Yes. And it is part of the 'bro code' but it depends on the situation and length of time. I had it happen twice, first was a friend who got with my ex about a month or so after we broke up, I wasn't too happy about it. But I got over it. Then another friend who hooked up with my other ex about a year or so after we broke up, they didn't tell me because they thought I would be upset, but I found out anyway and I didn't care at all. It was a year, and I knew how she was. I didn't blame my friend for that one bit.

I've got lots of experience with this. I could give two shits about an ex. Who cares? they broke up. Smash at will.
Sisters are a little different. My experience is all with younger sisters. Only once was the brother cool with it. But I also dated the younger sister of a guy I really didn't like. It was funny as frick to me that he would get so mad. He'd come in and I'd be stretched out in his sister's bed, just cheesing.
Now, a trickier pull is an ex-girlfriend's sister. They really hate that. But it can be done.
To me, the only women that are off limits are ones that are currently married to your friend.

All are fair game. Best friend in high school had a pretty fine little sister, after a concert she had tagged along to we hooked up. He found out and wasn't too upset, just asked that I didn't tell people and ruin her name.

My sophomore year at LSU my roommate came home with me one weekend. Met my sister - a senior in high school and pretty hot. He asked me if I had any problems with him dating my sister. I told him no. They went together for a few months and then parted. Never talked with either one about their relationship.
