Mine is similar except I don't really have a distinction between bad and horrible service in terms of what tip I leave. The bad service gets nothing, the horrible (to me, implying very rude or some other unacceptable behavior) gets nothing and I speak with the manager or owner.

Between 18-20% unless it's a buffet where the wait staff really isn't doing anything.
I was in Dallas for work a few months ago and a co-worker and I went to Twin Peaks to watch some football. Two older gentleman were sitting beside us and were obviously regulars at this establishment. When the guy got his check, he filled out both copies of his receipt. One was a 20% tip (5 dollars) and the other was 100 dollars. When the waitress came back he held them up and asked which one she'd rather have. Then he proceeded to tell her he needed her number if she wanted the hundred, which she obliged. It was pretty sad on both accounts

I sometimes pre tip a 20 dollar bill if I am with a bunch of rowdy friends. It most of the times leads to waiter giving us extra attention. Also leave normal tip at end.

The tipped wage has not been risen in louisiana since at least 1990. Its still 2.13/hour

Fake number. Most girls will literally never give their number out while at work. A better strategy is to ask them where they frequent after hours then go from there. If they are normal service industry then they will be obliterated 30 minutes after getting there so your chances are much better

I usually just double the tax or sometimes 20%. I round my total to the nearest dollar
Example:
Meal: 57.84
Tax: 5.49
Total: 63.33
double the tax would be 19% of the meal or 11.00. I might do 10.67 or 11.67 if they were good. Total would be 75.00 even (Im a nerd and do this when pumping gas too)

My tipping generally is a better % when the total is lower... but there is no rhyme or reason.
$10, I tip $5
$15, I tip $5
$17-$20, I tip $10
$30, I tip $10
$40, I tip $10
$50, I tip $13-$15
$60, I tip $15
$70, I tip $20
$80, I tip $20
$90, I tip $20
$100, I tip $30
.. and then 20% with maybe a few extra dollars on top from there.
No rhyme or reason. Just what feels right in my messed up little brain.
All of the numbers above can be larger if the person is super sweet/made friends with us. They are pretty much never lower than what is listed above.

That's either a lie or the standards are embarrassing low so that it's impossible to have bad service. I'm guessing it's the latter.

What? I've only had truly bad service once in my life.
Your standards are probably too high. Guarantee you're looking for shite to care about instead of enjoying the company you're with and not considering that these are humans waiting on you.
I am extremely polite and overly nice to servers. It is pretty fricking obvious I'm going to tip them well because of that. Maybe you're short and rude? Nothing worse than, "Hi how are you doing tonight?"... "DIET COKE".

That's the tip credit , they make at least minimum wage if not more. Most Servers should make about $15 on low end and it goes up from there. Obviously on how busy the restaurant is and what the average ticket price is

Good service= don't let drinks run dry, brings food timely, checks frequently but not bothersome
