No problem calling real PhDs doctor. Dr of Ed on the other hand...

I did a PhD in Engineering. There is no one size fits all for referring to PhD's and "Doctor" or not.
For example, my mother spent several days in the ICU and a few weeks recovering from stomach surgery at the end of last year. Consequently, I was spending a lot of time in hospitals. In no way, shape, or from would I expect or require anybody to refer to me as "Doctor" in that environment.
Now in my professional life, I work in the research environment where I am competing for contracts that ultimately pay the bills and provide for my family. So when a potential client is soliciting research companies to solve problems related to my dissertation, research experience, etc., you had better believe my PhD is going to be listed front and center on any and all proposals. I'd be an idiot to do anything else. Whether or not anybody decides to "Doctor" me or not in conversation, I have zero fricks to give.
In my everyday life, I again give exactly zero fricks if I'm called "doctor" or not. Unless I'm talking to Time Warner Cable. I will interrupt those mother frickers every time and make them call me "doctor".

....there is this one British cat that hangs out in a phone booth. Calls himself "the doctor" but I'm skeptical about his credentials.
