Leaving a phone and towel on an unused piece of equipment while you walk around the gym, and have no clue who's it is to ask to move it.

Not racking your weights
Occupying two pieces of equipment. There’s this one a-hole (fat as shite) at the neighborhood gym that loads up plates on bench press and takes up a squat rack to do deads and then takes time to shoot the shite while bouncing between the two. Frustrating as frick, even if I’m not needing to use either.

Lot of good things mentioned but fortunately do not encounter them much at my gym.
For the squat rack topic, we have 5 indoors and 3 outside. On a nice day or early mornings, I know there will be some available outside and will plan part of my workout to be outdoors.
