Before the railroad, the vast majority of people never travelled more than 20 or 30 miles from where they were born. There were exceptions for colonists, settlers, soldiers on campaign, but for most people that held true.

And with lower population densities along with the fact that many human societies, at least prior to the enlightenment, were extremely clannish and xenophobic - if you wanted to get married, at a certain point, you knew you were marrying a distant cousin (at best).
