This tells me NOBODY checked the intersecting traffic flow before proceeding. The small car on the right and the white van in the middle both had clear shots at oncoming traffic but the van on the far left had their view blocked by the van in the middle lane and he was just reacting to the others proceeding forward.
I find it hard to believe that at least 1 of those 2 drivers couldn't see what was about to happen. They were wiped out with the quickness.
