I know I do it all the time personally - doesn't matter male, female, etc.

I always do, no matter who is behind me. If it’s a woman I may hold a little longer. If there’s someone a little too long I’ll at least throw/shove the door open to assist them.
I catch myself almost always looking back when opening a public door. I guess it’s just habit to see if someone is coming behind me.

Yes always. I also have a problem that if they don't acknowledge it with a thanks, I say you're welcome, a little too loud after.

Yes for anyone, if they are within 16 feet of me. Otherwise it just gets awkward

I was raised in south Louisiana. If I am arriving at a door at the same time as a gentleman, I slow down assuming he will open it for me. If it's another lady, and I arrive first, I will open the door for her.
ETA I will speed up to get to the door before an elderly person so I can open it for them. Or people with their hands full.
This is the Way. I can't imagine living somewhere where people don't follow door etiquette.
