I think I agree with this. You can pick up on some thought or strain. If the person seems to be working really hard on what seems a simple concept or maybe just looks lost then you can make some (probably unreliable) simple judgement about his intelligence.

Not sure of a look, but mannerisms for sure. Fidgety, slightly clumsy, very fast talking etc etc. My cousin who made a perfect score on the SAT and a Cal Tech physics grad has all those traits, and I once asked him if it was me or if those mannerisms seem common in extremely intelligent people, and he agreed with me and said his classmates def were like that as well
