I'm not saying this to argue that I should be making 200K as a resident. I went into this knowing what I was signing up for.
Healthcare professionals need to take back control of our industry. Why is it that the discussion about healthcare reform is always between two options? Its always universal government-run healthcare vs private insurance-run healthcare? frick both of them. We need to kick them both out and captain this ship ourselves. Cutting out these middle-man leeches could save shittons of money in healthcare.
You have no idea what the reps do before and after the case. Doctors are always complaining about the extra work that isn't actual patient care the reps take care of a lot of organization and planning
You are coming off as kind of a jerk. Hopefully when you start your practice you realize that it doesn't benefit you to be that way. If you work with your reps they can be pretty helpful to your practice.

It's clear that he needs supervision and should not be able to post without more oversight. I guess that's to be expected since he has less than 400 posts so far. I'm thinking 8-10,000 should be the minimum before he is let loose on the general public.

Would you feel the same about paralegals being able to become full fledged lawyers after working an "appropriate" number of hours under one and not having to pass the bar? Same difference. Free market and all. I mean, if the the customer is ok with the legal counsel one could provide, would that not be in their best interests?

You're judging the end result (through your own lens - without medical training) and ignoring what's going on inside the calculator itself. Fancier computers (more training, higher test scores, more wiring) can do more and do it more successfully. Primary care is not just abx, steroid shot, abx, steroid shot, repeat. Clearly you don't know anybody in medical training

Another misconception is that NP's create cheaper care. Wrong!!! Most of their visits are visits that we used to treat over the phone free of charge. If they were the solution, costs would be decreasing instead of increasing. It's all based on a fallacy of cheaper care. What you will see is more visits and more tests and more referrals.
But what do I know?
Do what y'all want, but you've been warned. And I'm not angry by any means. I'll just keep hiring NP's. I'm not worried about mine leaving or competing with me. They only ones that will go out on their own are the arrogant/reckless ones that have no clue what they don't know. The market will sort things out. I'm hoping nobody else will go into family practice. From a business standpoint, it only stands to benefit me. I understand the need and demand for a good FP. The less there are of us that can provide the services we do, the more we can demand for our services.
I'm already seeing it as there are no internist left in our area, virtually no peds, and very few gyns. 
But what do I know?

very true.... and yes, while it may serve a broader patient base, and even alleviate the "gimme" cases from MD's, so they can focus on more advanced, complex cases, it will end up increasing, not decreasing HC costs... the way modern medicine is now, it's inevitable....
there is no slowing the machine at this point... you either blow it all up and start over, or shut the frick up... that's basically what politicians need to understand...
