People buying street drugs better start using Fentanyl test strips. They're very accurate and cost 1 or 2 bucks each.

But coke and fentanyl are opposite highs and it will kill the user. Seems bad for business to kill your customers and at the same time it causes fear among other users who may stop using which further affects business
Educated guess? It's almost entirely unintentional.
These aren't PhD laboratory scientists cooking products in state of the art facilities with rigorous QC checks throughout the process, and even those trained professionals make mistakes.
Somewhere in the chain, a lab or distributor (or both) is cutting cocaine while also processing heroin with fentanyl. Equipment doesn't get properly cleaned, chemicals are improperly stored, labeled, etc... whatever you can imagine in terms of terrible protocol, it's probably happening in a clandestine, Mexican laboratory somewhere.
One small mistake and you have a hotspot in your batch of coke, and there's no reversing it. Even if they were aware of the mistake, these people don't have much of a conscience and there could be harsh consequences for tossing a batch.
It's just my conjecture but it makes sense given the ODs tend to be in isolated groups. They hit a hotspot in their blow, probably opioid naive as well, game over.
